Who We Serve
Our programs are designed to support individuals with developmental and intellectual disabilities who choose to live in their homes and communities.
Developmental Disabilities
Includes individuals with autism, cerebral palsy, Down syndrome, and other qualifying conditions.
All Ages
We support children, adults, and seniors who meet program eligibility requirements.
Community Living
Services are provided in the comfort of home and within the community.
General Eligibility Criteria
To be eligible for our services, individuals generally must:
- Have a diagnosis of a developmental disability or intellectual disability.
- Require assistance with activities of daily living.
- Live in a private residence (own home, family home, or apartment).
- Meet financial eligibility requirements for Medicaid or program funding.
- Choose to receive services in the home and community instead of an institution.
